As far as i know (Ryan, correct me if i'm wrong..) :
The process is a random one, and is affected by the players ranks of the previous month.
The higher rank u had previous month, the more u have chances to be selected by the random process when creating a new table. Think about the rank is giving a percentage of being selected.
Some tables are created at the launch of the server, the next ones are created as needed (when others are filled up) using the same random process.
Moreover, there are 'class' of rankings which correspond to types of tables, for example a top 5 player will give his name to a $1000/$2000 table, never to a $25/$50..
This system is pretty good, even if u r ranked 40th, u still have a chance to get a table with ur name, with a little luck..
